ADMISSIONS PROCEDURES

INCOMING FRESHMEN 1st SEMESTER ADMISSION FOR A.Y. 2024-2025 GUIDELINES AND PROCEDURES

STEP 1: Accomplish the Application Form

Accomplish the College Application Form for Entrance Examination and create a CGHC student account via the automate.cghc.edu.ph Parents/Students Tab.

STEP 2: Pay the non-refundable and non-transferableAdmission Processing Fee of five hundred pesos  (₱500.00)

The mode of payment only include:
a.    On-site Payment- Proceed to the CGHC Accounting Officefor payment.Note: We do not accept bank transfer or
        online payments. Payments made online will be forfeited.
b.    Upload your proof of payment on your CGHC student accounts. (Read the College Admissions Guidelinesfor your reference in uploading your proof of payment)

STEP 3: Submit your Admissions Requirements at the CGHC Admissions Office and secure a Test Permit

Please refer to the requirements below:
1. Accomplished Application Form
2. Photocopy of PSA Birth Certificate
3. Certificate of Graduation/Certificate of Candidacy for Graduation/Certificate of Enrollment
4. Three (3) pieces identical and recent 2x2 colored picture with white background (wearing white polo with collar)
5. One (1) piece long ordinary folder
6. One (1) piece long plastic envelope
7. Two (2) pieces long brown envelope

STEP 4: Take the Online Assessment on your CGHC student account after your payment has been verified

Applicants who have uploaded their Processing Fee receipt may take the Online Assessment (Online Exam) on their student accounts before they take the Chinese General Hospital Colleges Entrance Examination (CGHCEE).

STEP 5: Take the CGHC Admissions Test

Releasing of results will be announced by the Proctor/s. Results can be seen on your student accounts and you will also receive a confirmation email.

STEP 6: Pay the non-refundable and non-transferable Reservation Fee of three thousand five hundred pesos (₱3,500.00)

Students who passed the CGHCEE are required to pay a non-refundable and non-transferable fee of ₱3,500.00 (includes reservation fee of ₱2,000.00 and medical examination fee of ₱1,500.00) over the counter at the CGHC Accounting Office only.

Note: We do not accept bank transfer or online payments. Payments made online will be forfeited.

STEP 7: Take the Medical and Physical Examination

Proceed to the Admissions Office to secure a medical examination schedule. Chest X-ray, HBsAg screening and anti-HBS titer test results will be automatically forwarded to the Admissions Office. Students are required to personally get their drug test results.

Collect all results at the CGHC Admissions Office and proceed to the School Clinic for physical examination.

STEP 8: Submit the complete enrollment requirements at the CGHC Admissions Office

Please refer to the requirements below:
1.    Two (2) copies of Student Information Sheet
2.    Original and photocopy of Senior High School Report Card (SF 9)
3.    Original and photocopy of recent (dated on or after graduation) Certificate of Good Moral Character with school dry seal
4.    Original and photocopy of recent (dated on or after graduation) Certificate of Graduation with the specific track and strand taken (Note: This is not the DIPLOMA)
5.    Original and photocopy of PSA-Authenticated Birth Certificate
6.    Original and photocopy of Medical Clearance Form issued by the College Physician

STEP 9: Enrollment (Kindly wait for further announcements)
